Why is poor performance only available when the player gets a 5 or less rating? Most annoying thing ever!

It's supposed to be available for 6 or worse but is currently bugged so it doesn't work all the time.  Regardless, on your screenshot, the player's last game was a 7 so that's something you can discipline.

If your players aren't performing well then switching formations or roles turns out to be far more effective than disciplining players in my opinion.

Why would a rating of 6 be considered bad? That's slightly above average, surely disciplining a player/players for that would lead to poor morale.

I thought so as well and that's how it is in real world football.  But as far as SI and the match engine are concerned, getting a 6 means the player didn't have a good game.  You should always aim and expect for your players to get ratings of at least 7 in matches.
