Chat When To Sub Players?

When do you guys usually substitute your players, like at what condition or rating, i usually do it when they hit like 80%,  and a rating of 5 at like 60-70mins...

If the game is going to plan I will sub players who are sub 80% condition around 65 mins.

If things aren't going to plan I'm quite happy to make tactical subs at half time.
